Hi all this might sound a little weird but ive never created a form before, im totally self taught and only learn things when i need to.

The form looks good so far. If I would have more time I would create the php file for you, but according to the current circumstances I would redirect you to this tutorial.

I had to shy away from things like this a while ago but I'm sure you'll do a good job with the form and get precisely how you want it look and work.

If not, may I reccomend Form Spring to you.
It works a treat, however there is now spam prevention!

Hi,guy.I'd like to give you some help.
I'm a web designer.[FONT='Verdana','sans-serif']Noticed that there are many people having trouble with writting good quality XHTML/CSS/JS code, we think you may have interests in the one of the services we offer(such as PSD to HTML).
You can find us easily by search keyword (We are No. 1 on the result)

    Good luck.:)
